Analysis of Patients with Acute Ischaemic Stroke Treated Via Endovascular Approach

Resumo

Introdução: O Acidente Vascular Cerebral (AVC) é uma das principais causas de morte e incapacidade no mundo, ocorrendo majoritariamente em adultos de meia-idade. Dentre as diversas classificações, o isquêmico é o mais frequente e mais recorrente. Assim, alguns fatores de risco devem ser levados em consideração, devido à alta taxa de morbimortalidade, além do adequado tratamento. Objetivo: Quantificar a melhora clínica obtida em pacientes com diagnóstico de AVC isquêmico tratados por trombectomia mecânica em um serviço de referência em neurocirurgia. Métodos: Trata-se de um estudo observacional retrospectivo baseado na análise de dados no período de outubro de 2020 a dezembro de 2024 em serviço de neurocirurgia endovascular. Resultados: Houve 52 pacientes com diagnóstico de AVC isquêmico, os quais realizaram o procedimento de trombectomia mecânica, sendo igualmente do sexo feminino e masculino, com idade entre 20-89 anos. Ademais, um único tratamento foi suficiente para a maioria dos casos e com escala TICI (Thrombolysis in Cerebral Ischemia) predominantemente 3. Conclusão: O tratamento endovascular de pacientes com AVC isquêmico com o uso de trombectomia mostrou uma boa aplicação e ótima evolução dos pacientes. Demonstrando por meio da escala TICI uma recanalização completa com reperfusão normal dos ramos.

Abstract

Introduction: Stroke is one of the leading causes of death and disability worldwide, occurring primarily in middle-aged adults. Among the various types of strokes, ischemic stroke is the most common and recurrent. Therefore, given the high rate of morbidity and mortality, certain risk factors must be considered, in addition to appropriate treatment. Objective: To quantify the clinical improvement observed in patients diagnosed with ischemic stroke who underwent mechanical thrombectomy at a referral neurosurgery center. Methods: The study is a retrospective observational study based on an analysis of data from October 2020 to December 2024 in an endovascular neurosurgery unit. Results: There were 52 patients diagnosed with ischemic stroke who underwent mechanical thrombectomy; the group was equally divided between men and women, ranging in age from 20 to 89 years. Furthermore, a single treatment was sufficient in most cases, and the TICI (Thrombolysis in Cerebral infarction) score was predominantly 3. Conclusion: Endovascular treatment of patients with ischemic stroke using thrombectomy proved to be effective and resulted in excellent patient outcomes. The TICI scale demonstrated complete recanalization with normal reperfusion of the branches.
